IF YOU ARE A FIRST YEAR MEMBER, YOU WILL BE REQUIRED TO UPLOAD A COPY OF YOUR CHILD'S BIRTH CERTIFICATE.

No practice particpation is allowed without it.

you will need to upload this form and a participant birth certificate in Step 2
